YOU, the LENDER is responsible for any delays in the certificate request process.  Be aware of the process and plan accordingly!

This is a form for LENDERS ONLY !!! 

***PLEASE DO NOT complete this form if you are a real estate agent, title company- anyone other than a lender- CONTACT THE LENDER TO MAKE THE REQUEST

HOMEOWNERS/BORROWERS—  STOP– DO NOT COMPLETE THIS FORM– IT WILL BE DELETED.
FOLLOW THESE DIRECTIVES BELOW:

IF You are Purchasing a Condo in an Association, refinancing or getting an Equity Line your LENDER MUST use this form and make the request on your behalf!!
**If you received a letter or email from your lender/mortgage company- requesting Proof of Insurance for your Condominium Association please call (727) 442-0012 extension 206.

*** LENDERS  *** BE ADVISED – REQUESTS WILL BE COMPLETED AS OUTLINED BELOW.  NO EXCEPTIONS WILL BE MADE.  Do not contact Turner Insurance or the Property Manager, they will NOT assist you and refer you back to this form!

ALL REQUESTS ARE COMPLETED DURING THE FOLLOWING BUSINESS HOURS ONLY

MONDAY – WEDNESDAY – THURSDAY – 9AM – NOON ONLY (EASTERN TIME)  (excludes holidays)

We will not reply to ETA questions.  You will get your documents IN 7 BUSINESS DAYS (as referenced above)

If you need your documents sooner than the allotted 7 business days (as referenced above), Select the Expedite option on the form below.

If you choose to expedite, the fee to Expedite is $125.00, and MUST be paid with a credit card.  NO EXCEPTIONS!

Once you submit the request and request to Expedite, you will receive a reply from the processing dept with directives to pay the expedited fee.  Once the fee is processed, you will receive your documents within 2 business days (based on hours of operation)

BE ADVISED – If you choose to expedite the process, BE AWARE, we will not process a refund if you choose to use the Pay Now feature on the website PRIOR to receiving the instructions to make the payment.  The instructions are very precise and you must wait to receive them before proceeding to make a payment!

If you are unsure of the proof of coverage needed, please check ALL coverage boxes.  It is not the responsibility of The Turner Insurance Advisor Group, Inc to obtain correct and/or accurate information on your behalf.  The lender is responsible for errors provided on the certificate request form and any delays due to the errors.
